Evolve MD Wellness and Aesthetics Med Spa is a “Fee for Service” medical practice. We will collect all fees for our services from you at the time service is delivered. We do not file insurance and are considered Out of Network for ALL insurance plans. If needed, we can provide you with a detailed receipt that you can submit to your insurance provider for reimbursement.
A full blood panel will be done prior to your first visit to review with you during your initial consultation. Lab work can be done at the outpatient lab of your choice and is covered by most insurance companies. Lab work typically takes 5 business days to return and will need to be scheduled prior to your initial visit.
Evolve MD Wellness and Aesthetics Med Spa has a discount with Quest and Labcorp laboratories for patients that do not have medical insurance or have a high deductible medical insurance plan. Please call the office for more information at 832-240-1032 or send us a message.
Some of the BHRT medications prescribes are available at commercial pharmacies and are covered by insurance. In some instances, a specialized compounding pharmacy will need to be used to make medications specifically for you. There are a few compounding pharmacies that do take insurance, but many compounding pharmacies do not accept insurance. The team works with several compounding pharmacies all over the Houston area. She will work with you to ensure the medication is as affordable to you as possible.
